Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community. Schedule: Monday – Friday, 8:00 a.m. – 4:30 p.m. Territory: Marlboro, Chesterfield and Dillon County In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services. Provide direct care and support, ensuring comfort and quality of life for patients. Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace. Educate patients and families, encouraging active participation in personalized care plans. Join the team in sharing call duties, being there when it matters most. Qualifications Current, unencumbered RN license. Minimum one year of RN experience in a medical‑surgical/acute care setting; hospice experience is a plus. Computer‑proficient, experienced with EMR systems. Current CPR certification where required by state. Valid driver’s license, vehicle insurance, and dependable transportation. Salary: $70,000 – $80,000 per year (average).
