Job Responsibilities

• Providing quality RN care in post-acute facilities
• Collaborating with care teams to develop care plans and identify the best options for residents
• Developing and adhering to individualized resident care plans
• Reviewing care options and tracking outcomes alongside clinical staff
• Administering appropriate medications and treatments to residents
• Properly setting up medical equipment and overseeing its use
• Collecting samples for lab analysis
• Engaging with residents and their family members to discuss and determine the best care options
• Ensuring resident histories and clinical interventions are thoroughly and accurately documented

Qualifications and Skills

• Valid state-issued RN license
• Graduate of an approved nursing program
• At least 6 months of experience in an RN or related role
• Strong organizational skills
• Ability to lift a minimum of 50 lbs
