Physical Therapist - Outpatient - Health and Wellness Center

Position Physical Therapist – Full Time (Day Shift, Monday‑Friday, no holidays or weekends). Employment with Hackensack Meridian Health. As a Physical Therapist at Hackensack Meridian Health, you will provide comprehensive, individualized, evidence‑based evaluation, intervention planning, and outcome analysis. You will coordinate, communicate and document care to ensure optimal functional independence and wellness, across inpatient (acute and rehab) or outpatient settings. Responsibilities Prepare for evaluations in accordance with department policies, obtaining patient history, demonstrating knowledge of diagnoses and researching unfamiliar ones. Administer and synthesize tests and measures to formulate diagnosis, goals, prognosis, and plan of care. Establish and modify treatment plan based on evaluation results and best practice; collaborate with interdisciplinary team, patients and family/support system. Initiate and complete discharge plans, providing education and training to patients and their support system. Provide functional training, including gait/locomotion training, therapeutic activities (bed mobility, transfers), community reintegration, ADLs/IADLs, work injury management, and equipment prescription. Apply musculoskeletal interventions such as therapeutic exercise, manual therapy, and joint mobilizations. Perform neurological interventions, including handling techniques, neuro‑developmental training, neuromuscular re‑education, cognitive rehab, behavioral management, spasticity management, and vestibular therapy. Conduct additional assessments such as cardiac rehab, medical monitoring, and splinting when needed. Collect and record patient care data and daily billing in compliance with regulatory and payer guidelines. Adhere to HMH organizational and managerial competencies and standards of behavior. Lift a minimum of 5 lbs., push/pull a minimum of 5 lbs., and stand for a minimum of 1 hour per day. Qualifications Required Graduate of an APTA‑accredited Physical Therapy program or equivalent. Excellent written and verbal communication skills. Proficient computer skills (Microsoft Office/Google Suite). Preferred Minimum of 1 year of clinical experience. Licenses and Certifications Required New Jersey Physical Therapist License. BLS/CPR certification by the American Heart Association. Preferred Specialty certifications. Compensation and Benefits Minimum annual salary of $92,144.00. Compensation may vary based on experience, education, certifications, geographic location, and cost‑of‑living adjustments.
