Responsibilities
- Assess home health patients in stable and emergent situations; document findings and communicate with the care team
- Develop, implement, and evaluate the Plan of Care; maintain up-to-date patient records reflecting visits, problems, plans, actions, and goals
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate these into the Plan of Care
- Educate patients and families about care, medications, home safety, and self-care; tailor teaching to age, condition, culture, and learning readiness
- Supervise and provide clinical direction to Home Health Aide/STNA/LVN to ensure quality and continuity of service
- Participate in on-call responsibilities and act as a liaison to patients, families, team members, and other healthcare professionals

Requirements
- Active Registered Nurse license in Maine (Required)
- OASIS experience (Required)
- Home Health experience: 2+ years; 1-2 years clinical RN care (Required)
- Homecare Homebase software experience (Required); CPR certification (Required); CHPN certification (Preferred); valid driver's license with reliable transportation (Required)
- Education: ADN or Nursing Diploma program graduate (Required); BSN (Preferred)
