Responsibilities
- Provide direct patient care to residents within the licensed practical nurse scope of practice
- Administer medications and treatments per physician orders and facility policy, document administration accurately
- Monitor residents’ conditions, document changes, and report to the RN or charge nurse
- Assist with activities of daily living, promote independence, and uphold resident dignity and comfort
- Maintain a safe, clean, and therapeutic environment; enforce infection control and safety protocols
- Collaborate with the interdisciplinary team to support care plans and resident outcomes

Requirements
- Active LPN license in Vermont (or eligibility for licensure in Vermont)
- Current Basic Life Support (BLS) certification
- Relevant experience in long-term care or skilled nursing settings preferred
- Graduate of an approved practical nursing program
- Ability to work 40 hours per week and adapt to day and/or evening shifts in a travel assignment
