Wayside Youth & Family has an open position for a Medical Case Manager on our vibrant Wayside Campus providing clinical services to youth and families. Wayside provides a warm work environment with management that is team-focused and places value on healthy work dynamics. This is the perfect job for folks who like to be busy, creative and offer innovative care to youth and families. Responsible for the front-line medical care of residents providing a safe and therapeutic milieu while promoting appropriate health education. Essential functions include providing transportation to medical appointments, facilitating medication oversight and education, and working with the staff team to ensure the safety of the program structure while striving for identified standards of behavior and providing positive role modeling for youth. Medical Case Managers are required to pass MAP certification training to administer necessary medications. Salary Range: $23.00 - $24.00/hour + sign-on bonus (half paid after 6 months of employment and the other half paid after one year of employment/pro-rated for part-time).

What You Would Do:

• Administrative: Responsible for medication transcription & consents. Count & order all required medications. Assist staff with the medication portion of intakes & discharges. Obtain immunization and PCP records.
• Care Coordination: Coordinate appointments with outside collaterals.
• Clinical: Nursing on call on weekends for psychiatry assistance. Document all medical needs, occurrences, and communication in appropriate locations including EMR.
• Training/Consultation: Conduct medication training for Residential & Day staff. Collaborate with programs for nursing needs including health education/awareness groups.
• Compliance: Document medication errors.
• Customer Service/Client Interaction: Maintain professional relationships with collaterals and guardians.

Minimum Requirements:

• High School degree or equivalent.
• At least 6 months experience in residential services and/or medical field.
• MAP certification preferred.
• Valid driver's license and access to a car.

We will train you in: Motivational Interviewing; Attachment, Regulation and Competency Model (ARC); Courageous Conversations; MAP, CPR, and First Aid.
