As an Integrated Health Manager, you will l ead care delivery and clinical operations transformations that improve throughput, utilization, and frontline performance for providers and payers. Y our primary responsibilities may include:

• Design and scale preventive, chronic, and complex care programs to close care gaps - Lead care coordination and transitions-of-care initiatives across payer, provider, and community settings - Support value-based care strategy, operating model design, and performance management

Travel

• As needed, could be up to 80%

Here's what you need:

• Minimum 5 years of consulting experience with healthcare payer and/or provider clients

• Minimum of 2 years of experience in population health, care management, or value-based care programs

• Bachelor's Degree

Preferred Qualifications:

• Experience supporting population health platforms or care management technologies

• Exposure to Medicaid, Medicare Advantage, or ACO operating models

• Background in health equity, community-based care, or behavioral health integration

• Strong understanding of longitudinal and cross-sector care models

Compensation at Accenture varies depending on a wide array of factors, which may include but are not limited to the specific office location, role, skill set, and level of experience.
