Additional production compensation is earned by meeting established productivity and performance metrics as your patient panel develops and varies based on individual performance. Evaluate and manage acute, chronic, and complex geriatric conditions, including frailty, dementia, cardiovascular disease, diabetes, and COPD. Manage symptoms, pain, medications, and quality-of-life concerns. Develop and regularly review individualized treatment and care plans with patients, families, and caregivers. Prescribe medications and treatments within scope of practice. Order and interpret laboratory testing, imaging, and other diagnostic studies as clinically appropriate. Educate patients and families about treatment options, advance care planning, hospice, and supportive services. Communicate and collaborate with patients, families, caregivers, facility staff, specialists, and interdisciplinary care team members. Coordinate specialty care, home health, therapy, hospice, ancillary services, and safe transitions between care settings. Maintain accurate and timely EMR documentation, including completion within 72 hours of each visit. Provide coverage for other practitioners within the care group as needed. Maintain compliance with organizational policies, professional standards, and regulatory requirements. Participate in continuing education and professional development. Who Thrives at AMG?
