Contract | Full-Time | Days | 5x8-hour shifts | Expected 40 hours/week

Key Responsibilities

• Evaluate and treat patients in an outpatient physical therapy setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to support patient progress and care goals
• Maintain compliance with facility credentialing and documentation requirements prior to and throughout the assignment

Required Qualifications

• Active Physical Therapist license — a permanent Oregon state license is required; compact licensure may be accepted at start but permanent Oregon licensure must be obtained promptly
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing requirements before assignment start

Preferred Experience / Skills

• Experience working in a high-volume outpatient or clinic-based environment
• Strong documentation skills and familiarity with electronic medical records
