Licensed Practical Nurse (LPN) - Full Time

Assignment Duration

This is a 13-week contract position.

Schedule

Shift: 3:00 PM - 11:30 PM, 5 days a week, including Every Other Weekend and Holidays.

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit, particularly ensuring the needs of adult and geriatric patients are met. Under the supervision of a Registered Nurse, the LPN will provide both direct and indirect skilled nursing care while maintaining a collaborative relationship with other nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions, consistently communicating findings to the RN and other members of the interdisciplinary team.
• Contribute to the development of age-appropriate and culturally relevant care plans, involving residents/families and the healthcare team.
• Recommend revisions to care plans based on evaluations and indicate the need for referrals as necessary.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring or orientation of staff.
• Ensure safety and cost-effectiveness in all planning and delivery of care, providing leadership as directed by the RN.
• Be aware of occupational exposure to blood-borne pathogens and follow all safety protocols.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure
• BLS Certification (AHA or RQI only)
• Minimum of 1 year of experience serving in an LPN capacity at a long-term care facility
