Certified Nursing Assistant

Position Summary The Certified Nursing Assistant is responsible for oversight of Personal Care Aides (PCAs), Direct Support Professionals (DSPs), and caregivers providing services in home care and group home settings. The Certified Nursing Assistant helps ensure services are delivered in accordance with: ? North Carolina Medicaid Clinical Coverage Policies (including Personal Care Services, Supported Living, and applicable waiver services) ? NC Division of Health Service Regulation (DHSR) requirements ? NCTracks documentation and billing standards ? Agency policies and procedures ? HIPAA and client rights regulations The Certified Nursing Assistant provides field supervision, quality monitoring, caregiver support, and reinforcement of person-centered care practices Primary Responsibilities 1. Caregiver Supervision and Support ** **? Support and monitor Personal Care Aides and Direct Care Staff. ? Conduct routine supervisory visits to client homes and group homes. ? Observe caregiver performance and adherence to care plans. ? Provide coaching, guidance, and additional training when needed. ? Assist in verifying staff competency in ADLs, infection control, and safety practices 2. Service Plan Implementation ? Review service authorizations and care plans. ? Ensure caregivers follow individualized, person-centered plans. ? Monitor that services remain within authorized hours. ? Report concerns or discrepancies to the Agency Director. ? Assist with care plan updates as client needs change. 3. Documentation and Compliance Monitoring ? Review daily notes and service documentation for completeness. ? Ensure documentation meets Medicaid and NCTracks standards. ? Verify EVV (Electronic Visit Verification) compliance when applicable. ? Promote timely submission of documentation for billing. ? Monitor compliance with incident reporting procedures. 4. Client Monitoring and Welfare Checks ? Conduct periodic client welfare visits. ? Assess client satisfaction and service quality. ? Observe for changes in health or functional status. ? Report concerns to the Agency Director or appropriate healthcare professionals. ? Ensure adaptive equipment is being used safely 4. Report incidents, injuries, or safety concerns promptly. ? Assist with internal reviews or investigations as directed. ? Support implementation of corrective actions when necessary. ? Help monitor clients who may require additional supervision. 7. Scheduling Support (If Delegated) ? Assist with coordinating coverage for staff call-outs. ? Help maintain adequate staffing in group home settings. ? Support continuity of care during emergencies or unexpected staffing changes. Minimum Qualifications ? High school diploma or GED required ? CNA certification preferred, but not required ? Candidates with experience as a PCA, DSP, caregiver, medical assistant, or other direct care role are encouraged to apply ? At least 6 months of direct care experience preferred (home care, group home, assisted living, or long-term care) ? Leadership, mentoring, or team lead experience is a plus, but not required ? CPR and First Aid certification (or ability to obtain within 60 days of hire) ? Ability to pass a criminal background check and OIG exclusion screening ? Valid drivers license and reliable transportation preferred Required C mpetencies ? Basic knowledge of ADLs and personal care procedures ? Ability to learn Medicaid documentation requirements ? Strong communication and interpersonal skills ? Ability to coach and support caregivers ? Understanding of infection control and safety practices ? Ability to recognize and report safety or risk concerns ? Basic computer and documentation skills Physical Requirements ? Ability to lift up to 50 pounds with assistance when needed ? Ability to travel locally to client homes and agency sites ? Ability to stand, bend, and assist with client mobility and transfers ? Ability to respond appropriately during emergencies Authority The Certified Nursing Assistant may: ? Provide coaching and performance feedback to caregivers ? Recommend additional training or reassignment when appropriate ? Report safety or compliance concerns to the Agency Director ? Participate in field competency evaluations ? Recommend disciplinary action to the Agency Director when necessary Performance Evaluation The Certified Nursing Assistant will be evaluated annually based on: ? Effectiveness of staff support and supervision ? Compliance monitoring results ? Documentation accuracy and timeliness ? Incident reporting and follow-up ? Client satisfaction feedback ? Overall contribution to quality and regulatory readiness
