Suvida Healthcare seeks a Guia to coordinate care for Medicare-eligible Hispanic seniors through a multidisciplinary primary care team. The Guia builds trust with patients and families, helps with social determinants, and arranges services in-home, in-clinic, and remotely to meet preventive, chronic, and acute needs. You will collaborate with physicians, nurses, case managers, and community partners, using EMRs and referral management tools. #J-18808-Ljbffr Suvida
