HHS Hunterdon Healthcare System Inc. , 215 Route 31 , Flemington , NJ M-F, 8am-4:30pm including on‑call weekend rotation , Variable shift Hiring Range: $60.00 Hourly Position Summary Provides medically prescribed clinical physical therapy in patients’ homes. Primary Position Responsibilities Facilitate and Coordinate Physical Therapy Care. Evaluates Progress Toward Attainment of Patient Outcomes. Documentation of Care. Employs Strategies to Promote a Safe Environment and Patient Health. Education of the Patient and Care Giver. Other duties as assigned. Qualifications Required: Bachelor’s Degree in Physical Therapy Preferred: Master’s or Doctoral Degree in Physical Therapy Minimum Years of Experience (Amount, Type and Variation) Required: Two years of clinical experience in a hospital, out‑patient, or rehab setting. Preferred: One year of Home Health experience. License, Registry or Certification Required: Current license to practice Physical Therapy in the State of New Jersey. Valid driver’s license. Current CPR certification. If not certified, must obtain certification within six (6) months of hire. Preferred: None Knowledge, Skills and/or Abilities Required: Excellent communication, time management, judgment and decision making skills. Computer functionality knowledge. Knowledge and skill in current PT practice. Preferred: Knowledge of home health care provision and principles.
