Home Health RN Case Manager

MJHS

MJHS Home Care is seeking a dedicated Registered Nurse to coordinate and deliver advanced clinical home health services. You will conduct patient assessments, manage medications, and educate patients and families in their homes, ensuring safe, effective care. The role requires an active NYS RN license, at least one year of acute medical-surgical experience, and a driver’s license with a car. Bilingual English/Spanish is a plus; a $15,000 sign-on bonus or student loan assistance is offered. #J-18808-Ljbffr
