Job Overview

Our organization is in search of an enthusiastic and mission-driven Lead Physician for Primary Care in Columbus. This role is pivotal in implementing team-based collaborative care and enhancing population health for our medically complex and socially vulnerable members.

We focus on providing care to individuals facing significant medical challenges and social vulnerabilities. Our approach transitions care from emergency rooms to a comprehensive, person-centered setting that emphasizes dignity, accessibility, and holistic support.

Key Responsibilities

• Deliver exceptional primary care to a select group of members, leading an interdisciplinary care team.
• Serve as the clinical leader alongside the Medical Director, offering clinical guidance to nurses, medical assistants, and other providers on various cases.
• Be an integral part of the leadership team, accountable for steering the market towards achieving quality, utilization, and engagement objectives. Participate in leadership meetings and represent the organization to community organizations, hospital systems, and payer partners.

Specific Duties

• Provide high-quality primary care, managing chronic conditions, acute exacerbations, and follow-up needs post-hospitalization or emergency visits.
• Collaborate with an interdisciplinary team, including behavioral health, nursing, social work, pharmacy, nutrition, and care management, to develop and adjust patient care plans.
• Mentor and coach physicians and Advanced Practice Providers on best practices in value-based and evidence-based care.
• Evaluate when patients can be treated safely within our organization and when higher-level care is necessary.
• Accept same-day, next-day, and urgent appointments for members requiring immediate physician or care management intervention to prevent admissions or readmissions.
• Monitor health trends among patients and proactively work to minimize ER visits, hospitalizations, and readmissions.
• Practice evidence-based medicine in line with Patient-Centered Medical Home (PCMH) principles, ensuring quality, prevention, and continuity of care.
• Develop and manage transitional care plans, tracking patients throughout the entire continuum of care.
• Achieve and maintain quality metric goals, utilizing data and electronic health record (EHR) tools to enhance performance and efficiency.
• Provide on-call coverage on a rotating schedule.
• Allocate approximately 70-80% of your time to direct clinical care.
• Perform additional duties related to chronic care and disease management as necessary.

Qualifications

• Possession of an active, unrestricted medical license in the state of employment.
• Preferred experience in a clinic, community-based organization, or inpatient unit.
• Strong skills in problem-solving, project management, and organization.
• Self-motivated and critical thinker with an interest in population health, healthcare utilization management, and the continuous improvement of our care model.
