Care Manager

We are seeking a compassionate and experienced Care Manager to join our team in Baton Rouge, LA. As a Care Manager, you will play a crucial role in ensuring the health and well-being of our clients by managing their care plans and providing exceptional support.
About the Role
As a Care Manager, you will be responsible for:
Coordinating and managing client care plans, including developing and implementing personalized care plans to meet each client's unique needs
Conducting initial and ongoing assessments to identify client needs, strengths, and goals
Developing and maintaining relationships with clients, families, and healthcare providers to ensure seamless care coordination
Identifying and addressing any gaps in care, ensuring continuity and quality of services
Monitoring client progress and making adjustments to care plans as needed
Responsibilities
Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
Communicate effectively with clients, families, and healthcare providers to ensure a seamless care experience
Identify and address any concerns or issues that may arise during the care process
Maintain accurate and up-to-date records of client care and progress
Collaborate with the management team to develop and implement new processes and protocols
Rotating on call
Assist with recruiting and training
