The role includes overseeing the review of patients’ medical records to ensure proper utilization of hospital services, assisting case managers in providing timely coordination of care and discharge planning, and ensuring screening of all admissions to determine the appropriate level of care. Supervising case managers and overseeing daily operations of the case management department to facilitate optimal financial and clinical outcomes is also a core duty. Additional responsibilities include fiscal planning and regulatory compliance, analyzing current systems and variances to identify opportunities for improvement, and promoting quality of care through collaboration with members of the interdisciplinary team. Education and Training RN licensure in the state where the hospital resides is required. Social workers may also be accepted in lieu of RN licensure, if they are licensed or hold a bachelor's degree in social work, as required and in accordance with state regulations. MSW, LSW, PT, OT, SLP, and RT licensure may be accepted in accordance with state regulations. Current BLS certification is required. BSN/MSN is preferred. Certification in an approved Case Management Program is preferred. Experience Three to five years of case management experience are required. Prior experience in IRF or LTACH is preferred.
