Licensed Clinical Social Worker

Position Overview Under the direction of the Director of Primary Care, provide psychosocial evaluation, crisis and trauma counseling, resource referral and case management services. Intervene with patients and families by applying critical thinking skills to monitor psychosocial status and issues that impact clinical progression, and develop transition or discharge plans, including facilitation of patient and family communication, coping, and decision making. Work as part of an interdisciplinary team. Responsibilities Monitor psychosocial status of patients, develop intervention plans, coordinate resources, and facilitate communication between patients, families, and the care team. Provide crisis counseling and trauma support. Manage case referrals and follow‑through, ensuring continuity of care and optimal transition or discharge for patients. Education and Experience Minimum Required: Master’s degree in social work from an accredited program, licensed as a Licensed Clinical Social Worker (LCSW) in New York State, and experience working with populations such as individuals with mental health concerns, families, or older adults. Preferred: 3 years of postgraduate experience. Licensure & Certification NYS LCSW required.
