Registered Nurse RN - Float Pool

MyMichigan Health

Summary

This Registered Nurse (RN) - Float - Night's position is a highly skilled role requiring advanced medical and surgical expertise to care for a diverse patient population at Midland and Saginaw Level II Trauma Centers, verified by the American College of Surgeons. The Midland facility serves as the hub of the hospital system, frequently receiving the highest-acuity patients from surrounding locations. Likewise, the Saginaw facility operates as a comprehensive Stroke Center, treating complex neurological cases from across the region.

This role demands a high degree of autonomy, critical thinking, and adaptability, as it involves floating between multiple facilities - often within the same work week. The night shift schedule further necessitates independent decision-making and proactive patient management to ensure optimal outcomes in fast-paced, high-pressure environments. This is a more complex and demanding nursing role than standard inpatient positions, offering a dynamic and challenging opportunity.

Responsibilities

(30%) * Regularly assesses patients/clients needs and provides skilled nursing services as prescribed by the physician authorized plan of care, and accepted standards of nursing practice.

(30%) * Provides direct resident care, including medication administration, treatments, call lights, assisting with transfers, responding to alarms, etc.

(20%) * Evaluates effectiveness of treatment plan and makes suggestions or participates in updating /revising the treatment plan in collaboration with the other team members, utilizing current information to improve patient care.

(10%) * Processes physician/provider orders and performs physician ordered care accurately, completely and in a timely manner.

(10%) * Follows up regarding medication changes and order clarifications.

OTHER DUTIES AND RESPONSIBILITIES:

Demonstrates the ability to quickly adapt to varied workflows, unit structures, and high-acuity patient populations across multiple Level II Trauma Centers while ensuring seamless transitions between medical/surgical and progressive care units.

Demonstrates proficiency in rapid clinical assessment and data interpretation, effectively communicating critical findings to physicians and making autonomous decisions, particularly during night shifts with limited provider presence.

Assesses patient and family learning needs, considering acuity, cognitive ability, and cultural/spiritual factors, and provides concise, individualized education to ensure understanding despite high turnover and shortened inpatient stays.

Participates in care conferences as assigned.

Evaluate patients' vital signs, laboratory data, and clinical presentation to identify emergency intervention needs, often in fast-paced, high-pressure environments where critical decision-making is essential.

Monitor high-acuity patients for signs of deterioration, including sepsis, shock, neurological decline, or respiratory distress, and initiate appropriate interventions with limited immediate physician presence during night shifts.

Administer medications intravenously, via injection, orally, or through gastric tubes, ensuring precision in dosage and continuous monitoring for adverse reactions, particularly in complex multi-system failure patients.

Perform complex pulmonary assessments, recognizing abnormal respiratory patterns or breathing difficulties that may indicate the need for urgent interventions, particularly in progressive care patients with advance respiratory conditions.

Assess and prioritize care for critically ill patients based on acuity levels, clinical pathways, and evolving treatment needs while efficiently managing high patient turnover in both medical/surgical and progressive units.

Monitor fluid intake/output, electrolyte imbalances, and hemodynamic stability in patients at risk for complications, balancing the needs of diverse patient populations across multiple units and locations.

Perform approved therapeutic and diagnostic procedures, including advance wound care ostomy management, heparin therapy protocols, and complex IV care, often requiring independent judgment during overnight hours.

Set up, monitor, and troubleshoot advanced medical equipment, including cardiac monitors, ventilators, transducers, and infusion pumps, ensuring optimal function in high-acuity settings with limited technical support during night shifts.

Assist physicians with high-risk procedures, including bronchoscopy, endoscopy, central line insertions, and emergency airway management, frequently in situations where rapid intervention is necessary.

Coordinate and oversee patient care provided by patient care technicians and interdisciplinary team members, ensuring efficiency in a high-volume, high-acuity environment where rapid adaptability is required.

Work closely with physicians, respiratory therapists, and other healthcare professionals to develop and continuously adjust treatment plans based on real-time patient assessments, especially in trauma and complex medical cases.

Act as a clinical resource and mentor for less experienced nursing staff, providing guidance and instruction on managing critically ill patients across multiple facilities and units.

Participate in patient care conferences and advocate for patients and families, ensuring emotional and psychosocial needs are met while managing the high demands of progressive and trauma units.

Identify patients at risk for nutritional deficiencies, psychological distress, or inadequate copying mechanisms, initiating early interventions to support overall patient recovery and stability in a fast-paced setting.

Maintain accurate, clear, and concise documentation of assessments, interventions, and patient progress ensuring compliance with regulatory standards and effective communication across multiple shifts and units.

Report significant changes in patient conditions, abnormal diagnostic findings, and treatment responses to the appropriate care team members, often making critical decisions with minimal supervision during overnight shifts.

Remain current on evidence-based practices, new medical technologies, and hospital protocols through continuing education and professional development, ensuring best practices are applied in high-stakes clinical scenarios.

Maintain proficiency in emergency preparedness procedures, ensuring readiness for mass casualty incidents, rapid code responses, and critical patient escalations across two Level II Trauma Centers.

Serve as an active member in professional nursing organizations and participate in ongoing training programs to enhance clinical skills, particularly those relevant to trauma and progressive care settings.

Assist with the orientation, training, and competency development of new staff, ensuring that all team members are equipped to handle the high-acuity patient population and fast-paced environment.

Provide post-mortem care, ensuring dignity and support for families while handling complex end-of life situations with sensitivity and professionalism.
