We are looking for compassionate, skilled, and dependable Physical Therapists (PT) and Occupational Therapists (OT) to join our team and provide quality, patient-centered care in the comfort of patients’ homes.

PHYSICAL THERAPIST (PT)
Provide skilled physical therapy services to help patients improve strength, mobility, balance, coordination, functional ability, and independence.

OCCUPATIONAL THERAPIST (OT)
Provide skilled occupational therapy services to help patients regain independence with activities of daily living, functional skills, safety, and adapting to their home environment.

RESPONSIBILITIES

• Evaluate patients and develop individualized treatment plans
• Provide skilled therapy services in the home setting
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers
• Promote safety, mobility, independence, and quality of life
• Maintain accurate and timely clinical documentation
• Communicate and collaborate with physicians and other members of the care team
• Provide professional, compassionate, patient-centered care

QUALIFICATIONS

• Current professional PT or OT license , as applicable
• Graduate of an accredited PT or OT program
• Appropriate professional credentials and certifications
• Home health experience preferred
• Strong clinical, communication, and organizational skills
• Ability to work independently in patients’ homes
• Flexible availability for scheduling
• Reliable transportation for patient visits
