POSITION SUMMARY:
The Black Infant Health (BIH) Program is meant to address the problem of poor birth outcomes and health disparities that affect Black women and birthing people and their babies. The BIH program features a standard set of services that features group intervention and social service case management to help reduce stress, build resilience, promote healthy behaviors, and increase social support. The BIH Community Health Advocate (CHA) will use active and passive outreach strategies, including referrals, direct outreach at community events, and media campaigns to ensure robust recruitment into the Black Infant Health Program. Will demonstrate strong relational skills to build trust and offer unwavering support and motivation to Black individuals during childbirth and parenting. The role also requires that the BIH CHA connect individuals with essential community services and resources and advocate for the coordination of healthcare and social support services for those individuals. This position supports Alameda County’s BIH Program Area 2, covering families who reside in Alameda County.
QUALIFICATIONS:- 1.Education: Minimum of Associate Degree. Prior work in healthcare, social services, community outreach, case management, or similar fields is preferred.
- 2.Experience: Demonstrated lived understanding of Black culture and history, with strong knowledge of family systems, community trauma, community organizing, and advocacy. Proven ability to collaborate effectively within multidisciplinary teams to support individuals, families, and communities. One to three years of experience working in community-based organizations, delivering culturally responsive services and fostering community engagement.
- 3.Group facilitation: Strong communication, active listening, empathy, and cultural responsiveness skills. As a facilitator, you must create a safe and supportive environment, build trust, encourage participation, and understand the social and health challenges affecting Black families to promote wellness and positive birth outcomes.
- 4.Certification: FHA/COL’s will participate, as needed, in training to receive the Community Health Workers certification (CHWs). FHA/COL’s will be expected to seek and complete other health professional development courses related to sexual and reproductive health, family planning, lay counseling, and other topic areas as appropriate. Doula certification plus.
- 5.Black Infant Health Ambassador: Fully embraces and supports the BIH Governing Concepts of cultural competence, client-centered, strength-based, and cognitive skill building approaches.
- 6.Cultural Competence: Demonstrates cultural competence and ability to operate in a culturally affirming manner. Knowledgeable about community organizations and community resources serving the Black community in their local jurisdiction. Demonstrates an understanding of the complex, interrelated issues and concerns contributing to health disparities affecting the Black community.
- 7.Knowledge of Public Health: Good foundation of public health knowledge as it relates to healthy equity, health education, social determinants of health and community resources.
- 8.Advocacy and Outreach Skills: Advocacy and outreach skills will be essential to promote health equity by implementing health programs and health policies that will benefit the community’s health through motivational training.
- 9.Professional and Communication Skills: Basic administrative skills (i.e. appointment scheduling, documentation, report writing, organizational skills). Sound data entry skills. Proficient phone etiquette. Excellent oral and written communication skills. Excellent attention to detail and ability to provide timely follow-up. Exceptional interpersonal skills and time management skills, ability to work effectively in a fast-paced environment, with rapidly shifting priorities and competing demand.
- 10.Computer Skills: Basic computer skills, and capable of using software and technology tools used to manage health data and reporting tools (Excel, Microsoft word, etc.). Basic administrative skills (i.e. appointment scheduling, documentation, report writing, computer skills, organizational skills). Experience in managing and tracking health data, navigating Epic and Tableau a plus.
- 11.Other: Should be able to remain calm and courteous in a high stress environment. Should be able to work independently with minimal direction.
- 1.Black Infant Health program recruitment, referral, and networking (Community Outreach Lead). Develops, maintains, and leads a site-specific BIH recruitment plan for multiple locations. Establishes a database of community agencies and creates relationships to obtain BIH referrals. Maintains relationships with medical and community service providers who are the primary referral sources for BIH. Develop partnership agreements to assist in providing referrals to the BIH Program. Conducts indoor and outdoor outreach activities on a regular basis; for example, conducting in-service training events and BIH Orientations for partnership organizations.
- 2.Navigation. FHA/COL will connect Black birthing and parenting individuals to California’s Enhanced Care Management Program, ensuring they receive comprehensive, coordinated care. Will assist patients with accessing ongoing medical care and social service support through medical care navigation and follow-up. The FHA/COL will comply with all navigation requirements established by BIH. The FHA/COL will serve as part of an integrated health care team to coordinate health services, assist with referrals, and ongoing care coordination.
- 3.Data collection, entry, and maintenance. Will input BIH case management and program data in a timely and accurate manner into the State data system and download program information from the MCAH- BIH-Efforts to Outcome (ETO). Maintain confidentiality and adherence to HIPAA regulations. Will use Epic electronic health record to update and maintain registry of patients that can follow patients based on panels belonging to individual primary care providers or disease/condition. Will retrieve information as requested, or necessary, and summarize data in preparation for standardized reports.
- 4.Community outreach and coaching. FHA/COL will participate in health and wellness coaching including activities such as counseling patients on healthy behaviors and providing patients with general support. Will conduct outdoor outreach to promote healthy lifestyles and behaviors to targeted populations.
- 5.Media participation. The COL will comply with all the tasks required by AC BIH including but not limited to update all local marketing and display materials/ Promote new BIH Website, social media pages, and/or apps at local provider offices and other outreach locations. Provide regular updates to the BIH Media Team on upcoming local recruitment/enrollment events, including additions or changes to events. Provide input to the BIH Media Team for the development of new standardized Statewide BIH marketing materials. Will contribute to creating social media content and participate in BIH Program media features, as requested. Recruit BIH participants and staff for media features, as requested.
- 6.Community engagement/advocacy. An FHA/COL will attend interagency, CDPH/MCAH-sponsored BIH Basic Training, and subsequent BIH Advanced Trainings.
- 7.Professional development. Will participate in a Professional Development Plan in accordance with the requirements of AC BIH.
- 8.Other Duties as Assigned. FHA/COL will attend and actively participate in training and client care coordination meetings and tasks as well as other duties as assigned.
- 1.Attends work regularly and punctually; is readily accessible during work hours to peers; limits absences from own duty stations and limits visits and activities which distract others from their performance of duties.
- 2.Willingness and ability to frequently travel to different Baywell clinics, Black Infant Health sites, and other community-based locations and to perform work and meet with providers, patients, and staff.
- 3.Works cooperatively with immediate supervisor, with department members, and with other Baywell Health staff persons, as required.
- 4.Treats all patients / clients / visitors with respect and courtesy.
- 5.Remains competent in skills required for position.
- 6.Some evening and weekend work may be required.
- 7. Maintains working knowledge about and in continued compliance with all those areas listed under item (5) throughout your employment with Baywell Health.
- 8.Provides care appropriate to the age of the patients / clients / visitors served.
