PT - Physical Therapist

Outpatient Physical Therapist Position

This is a 13-week contract position.

5x8.0 shift.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that promote effective return to community activities. This role also involves educating patients, families, students, coworkers, and self, while performing additional professional duties related to patient care and the development of discipline-specific therapy services and programs.

Responsibilities:

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records clearly, accurately, and timely, ensuring compliance with state, federal, and clinic policies.
• Provide care under signed and valid physician orders, adhering to insurance approvals as necessary.
• Participate in the student program, offering supervision and completing student evaluations.
• Develop an individualized professional development plan and pursue continuous professional growth and education.
• Maintain professional licensure in good standing and adhere to ethical standards and the discipline-specific professional code of conduct.
• Understand and implement organizational performance improvement initiatives.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state licensure.
