Physical Therapist - Full Time

Physical Therapist - Skilled Nursing Facility. Monticello, MN.

We are seeking a Physical Therapist for a full-time role at a skilled nursing facility in Monticello, MN. The clinician will evaluate, develop, and implement individualized treatment plans to improve patient mobility, functional independence, and quality of life. The position involves working with post-acute and long-term care populations and coordinating care with interdisciplinary team members. Minimum Requirements:

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active MN state license to practice physical therapy required at start.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience in skilled nursing, long-term care, or post-acute care preferred.
• Ability to manage caseloads, document thoroughly, and collaborate with nursing and therapy staff.
• Flexible schedule availability as the role requires adaptable shift coverage.
