Senior Community Care
Senior Community Care is seeking a Registered Dietitian to join our interdisciplinary PACE team supporting seniors living safely at home. The Dietitian will assess nutritional status, develop individualized care plans, and provide evidence-based counseling to manage chronic conditions, maintain function, and prevent malnutrition. Responsibilities include collaborating with physicians, nurses, therapists, and social workers; monitoring outcomes; documenting in the EMR; leading nutrition education for participants and caregivers; and contributing to quality-improvement initiatives in a compassionate, mission-driven setting.
