ABOUT TRENTON HEALTH TEAM

Trenton Health Team (THT) fosters cross-sector collaboration and implements innovative projects to improve health and well-being in the greater Trenton community. Founded in 2006 as an effort to address healthcare delivery and utilization among partners including St. Francis Medical Center, Capital Health, Henry J. Austin Health Center, and the City of Trenton Department of Health and Human Services, THT is now one of four State-designated Regional Health Hubs focused on addressing the social determinants of health and achieving equitable health outcomes

THT operates with a broad understanding of what health, health equity, and well-being mean for the Trenton region. Our work shifts in response to the needs of our community and of our partners, and is based on robust data analysis. Current priorities include chronic disease management, care management for community members with complex health needs, food and housing security, maternal and child health, mental health and substance use disorder, disability services, and neighborhood revitalization. Work in these areas is carried out with a wide range of stakeholders from across the region and across sectors, including those from the more than 100 organizations that participate in our Community Advisory Board.

POSITION OVERVIEW

Reporting to the Chief Operating Officer, the Director of Grants and Development will work across project teams to understand and provide input into strategic decisions about organizational priorities and funding needs. The Director will be responsible for identifying and coordinating responses to funding opportunities for Trenton Health Team priorities and projects, as well as identifying and sharing opportunities and supporting applications for funding relevant to the priorities and projects of Community Advisory Board participants and other partners. 

Individual contributions currently represent a small proportion of THT’s funding and we see the potential for growth. The Director will be an integral part of evaluating the viability of this new funding strategy as we explore how THT can build meaningful relationships with individuals who care about health equity. The Director will be responsible for guiding this evaluation and making recommendations for the possible development of a program to cultivate, solicit, and steward new major donor relationships.

Day-to-day responsibilities will include researching, identifying, developing, and coordinating responses to funding opportunities that support the work of THT and our partners; writing proposals for both unrestricted operating revenue and restricted projects; managing grant reporting requirements, including working with financial and program staff to ensure that reports are accurate and timely; cultivating and supporting individual donors, including acknowledging gifts and thanking donors; managing data systems to track grants and donations; and proactively remaining up-to-date about and engaged in THT’s and partners’ priorities, programs, and projects.

ESSENTIAL FUNCTIONS

• Oversee and coordinate the full range of activities required to identify, prepare, submit, and manage funding proposals to federal, state, county, corporate, and private funders and donors.

• Draft and edit successful proposals for funding using clear and compelling language, and assist other staff with proposal preparation. Create and manage proposal development timelines and task lists. 

• Develop, maintain, and steward private and corporate funder and donor relationships, as delegated by the COO.

• Understand THT’s institutional history and programs in order to represent its interests to funders, donors, and other stakeholders.

• Support cross-functional program planning, development, and implementation to help THT meet its strategic objectives.

• Cultivate strong relationships with project leads, with a focus on understanding and fostering programmatic alignment and collaborative funding opportunities. 

• Recommend and implement short- and long-range fund development plans that support THT’s values, mission, and priorities, including maintenance of a pipeline of relevant funding opportunities and evaluation of THT’s current and potential future individual giving program.

• Organize and lead go/no-go funding opportunity decision processes. 

• Support project leads in creating progress reports and assisting other staff with reporting to THT’s Board and leadership, funders and donors, and external stakeholders.

• Coordinate responses to partners’ requests for letters of support for their funding efforts.

• Provide ongoing support and training on proposal development processes to staff.

• Maintain current grant and donor records in databases, including grant tracking and reporting.

• Be an early and enthusiastic adopter of new technological data systems across the organization.

• Represent THT at informational meetings

ADDITIONAL FUNCTIONS

• Assume all other duties and responsibilities as assigned.

SUPERVISORY RESPONSIBILITIES

The Director may be asked to have staff supervisory responsibilities. In that case, the Director will:

• Carry out supervisory responsibilities in accordance with the organization’s policies and applicable laws.

• Be responsible for interviewing, hiring, and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

REQUIRED QUALIFICATIONS

• At least five years’ experience in successful grant writing and management from a wide range of funders for community-based nonprofits.

• Excellent written English and oral communication skills with the ability to deliver strong and compelling presentations to diverse audiences.

• Excellent organizational skills.

• Attention to detail.

• Demonstrated proficiency with common office software tools and digital tools for the management of grants and donor relationships.

• Experience of or openness to incorporating AI tools into the grant writing and management process.

• Strong project management skills.

• Demonstrated ability to manage multiple tight deadlines that require input from several colleagues and/or external stakeholders.

• Demonstrated understanding of data analytics, with a specific focus on measuring the impact of funding and reporting on grant metrics.

PREFERRED QUALIFICATIONS

• Bachelor’s or Master’s degree in a related field

• Familiarity with the Trenton community and/or other similar cities, including demographics, challenges, and key organizations that play a role in fostering health and well-being. 

• Experience delivering programs similar to those provided by THT.
