What You'll Be Doing As a Home Health Physical Therapist, you will evaluate patients in their homes and develop treatment plans designed to improve mobility, strength, balance, safety, and overall function. Your responsibilities will include: * Performing physical therapy evaluations and reassessments * Identifying mobility limitations and functional deficits * Creating individualized treatment plans based on patient needs and goals * Implementing interventions focused on strength, balance, gait training, and fall prevention * Teaching patients and caregivers exercises and safety techniques * Monitoring progress and modifying treatment plans as appropriate * Coordinating care with physicians, nurses, caregivers, and other healthcare professionals * Completing documentation in accordance with Medicare home health requirements What We're Looking For Successful candidates typically possess: * Current Hawaii Physical Therapist license * Minimum one year of clinical experience * Home health experience preferred but not required * Strong communication and patient education skills * Ability to work independently and manage a field-based schedule * Reliable transportation and valid driver's license * Commitment to providing compassionate, patient-centered care About Always Best Care Home Health Always Best Care is a growing Medicare-certified Home Health agency serving patients throughout O‘ahu.
