RN Case Manager - Home Health Location: Covington, Tennessee Company: Strong Force Staffing (RPO for Client Agency) Overview Welcome to Strong Force Staffing - Building Stronger Teams for a Stronger Tomorrow. Join our clients dedicated team in Covington, TN, as an RN Case Manager for home health care. You'll manage patient care plans and ensure the delivery of comprehensive home health services.

Responsibilities

  • Conduct comprehensive patient assessments.
  • Develop and oversee individualized care plans.
  • Coordinate care with interdisciplinary teams.
  • Educate patients and families on disease management.
  • Monitor patient progress and adjust care plans as needed.

Requirements

  • Certification: Licensed Registered Nurse with no restrictions.
  • Experience: Minimum 2 years in home health care.
  • Skills: Excellent clinical and communication skills. Strong organizational abilities. Proficiency in EMR systems.
