Responsibilities
- Conduct comprehensive patient assessments.
- Develop and oversee individualized care plans.
- Coordinate care with interdisciplinary teams.
- Educate patients and families on disease management.
- Monitor patient progress and adjust care plans as needed.
Requirements
- Certification: Licensed Registered Nurse with no restrictions.
- Experience: Minimum 2 years in home health care.
- Skills: Excellent clinical and communication skills. Strong organizational abilities. Proficiency in EMR systems.
