Assessment RN

The RN completes a full client assessment, reviews medical history, medications, functional abilities, and safety risks. The RN provides professional recommendations, updates or creates the care plan, and documents all findings according to agency and state requirements.

Responsibilities and Duties

• Perform client intake assessments
• Sign-off on client care plans for VA department (if applicable)
• Assign client care plans
• Assist with caregiver training and orientations
• + other duties
