Develop individualized treatment plans aimed at improving mobility, reducing pain, and restoring physical function. Set short-term and long-term goals for rehabilitation, considering patient progress and overall health. Collaborate with other healthcare professionals (e.g., nurses, occupational therapists, speech-language pathologists) to coordinate care. Administer a variety of physical therapy techniques, including therapeutic exercises, manual therapy, gait training, and the use of modalities like ultrasound, electrical stimulation, or heat/cold therapy. Maintain accurate and timely records of patient evaluations, treatment plans, progress, and any adjustments in care. Write progress notes and reports to keep healthcare teams informed. Communicate with physicians, family members, and caregivers about patient progress and modifications to care plans. Requirements for PRN (PT) Physical Therapist Licensed as a Physical Therapist in PA CPR certification (preferred). 1+ year experience in a Skilled Nursing Facility Compassion and patience when working with elderly or disabled patients.
