Position Overview BeeHive Homes is an assisted living facility caring for elderly residents. We need a new Care Coordinator in one of our homes. Our Care Coordinators help manage the day-to-day functions of the home. If you are compassionate, positive, have strong leadership skills, and are organized and creative, come join our team. We are seeking a highly motivated and compassionate Care Coordinator to join our team. The care coordinator will coordinate care services for individuals, ensuring they receive appropriate and timely treatment, support, and resources. The successful candidate will have excellent communication and organizational skills and should be comfortable working in a fast-paced environment. This role requires understanding and empathy for individuals dealing with healthcare challenges and the ability to advocate for their needs. Responsibilities Coordinate Care: Manage and coordinate care services for individuals by liaising between patients, healthcare providers, and other involved parties, ensuring delivery of comprehensive care across various settings. Report Needs to RN: With RN determine individual needs, goals, and preferences, collaborating with patients and the RN to develop personalized care plans that address physical, emotional, and social well‑being. Advocate for Patients: Act as patient advocate, ensuring their rights are respected and their voices heard, representing patients’ interests during interactions with healthcare providers and other stakeholders. Service Coordination: Collaborate with RN and administrators to organize treatments and follow‑ups, monitor delivery and quality of services, and assist in resolving any issues that may arise. Documentation and Reporting: Maintain accurate and updated records of patient interactions, care plans, assessments, and referrals, documenting outcomes with RN to evaluate effectiveness and identify areas of improvement. Requirements CNA certification with 2 or more years of experience. Previous experience in care coordination, case management, managing other employees or teams, or similar roles preferred. Strong understanding of healthcare systems, processes, and protocols, or willingness to learn. Excellent communication and interpersonal skills to establish rapport and maintain effective relationships with patients, families, and healthcare professionals. Ability to handle sensitive and confidential information with discretion and compassion. Exceptional organizational skills with ability to multitask and prioritize tasks efficiently.
