Physical Therapist (PT) – Outpatient Rehabilitation – Day Shift – Relocation Candidates Welcome

We are seeking a Physical Therapist (PT) to provide individualized rehabilitation services in an outpatient setting in Baker, Montana. The ideal candidate will be adaptable, open-minded, collaborative, and committed to helping patients achieve their functional goals through personalized treatment and education.

Approximately 95% of the role will involve outpatient care , with the remaining 5% supporting long-term care (LTC) and skilled swing-bed patients . The therapist should be comfortable treating a broad range of patients and working collaboratively with other healthcare professionals.

This is a 13-week contract with a strong possibility of extension.

Key Responsibilities

• Assess patients and develop individualized physical therapy treatment plans.

• Implement treatment programs designed to improve mobility, strength, function, and independence.

• Educate patients on exercises, rehabilitation techniques, and strategies to support recovery.

• Monitor patient progress and modify treatment plans to achieve functional goals.

• Provide therapy services primarily in an outpatient setting, with occasional LTC and skilled swing-bed coverage.

• Adapt treatment approaches to meet the needs of a diverse patient population.

• Communicate effectively with patients, families, and other healthcare professionals.

• Maintain positive working relationships with clinical and ancillary staff.

• Remain open to feedback and contribute suggestions to improve patient care and team performance.

• Active Physical Therapist license valid for practice in Montana.
• Minimum 2 years of Physical Therapy experience .
• Ability to treat a broad range of patients in an outpatient rehabilitation setting.
• Flexibility to provide occasional care in long-term care and skilled swing-bed settings.
• Strong clinical assessment, treatment planning, and communication skills.
• Collaborative attitude and willingness to adapt to the facility's patient-centered approach.

Schedule & Assignment Details

• Contract Duration: 13 weeks.
• Start Date: October 26, 2026 
• Shift: Monday–Friday, 8:00 AM–5:00 PM.
• Guaranteed Hours: 40 hours per week.
• Positions Available: 1.
• Work Setting: Approximately 95% outpatient and 5% LTC/skilled swing-bed care.
• Contract Extension: Strong possibility of extension.
• Holiday Coverage: Not required.
• Call: No call required.
