Position Summary Provide case management services, establish positive working relationships, and develop person‑centered plans. Meet state‑mandated requirements, secure ongoing support services, and ensure all services meet quality and compliance standards. Essential Duties Advocate on behalf of individuals and provide resource information. Educate individuals, families, and support staff on self‑determination and service coordination. Collect eligibility documentation for DDA services and recommend priority to DDA. Conduct comprehensive assessments, develop and update Person‑Centered Plans (PCPs), and write Self‑Directed Plans. Research, refer, and coordinate resources to achieve PCP outcomes. Travel to meet with support staff and conduct on‑site visits. Assist with transitions, paperwork, wafer follow‑ups, and documentation for medical assistance. Submit required recertification and investigation reports; apply for new programs and services. Communicate with providers to resolve client concerns and ensure compliance with SCI policies. Meet performance utilization benchmarks and maintain secure HIPAA records. Or bachelor’s in a non‑human services field with two years of experience in a human services field. Experience Preferred Experience working with people with developmental disabilities. Physical Demands Standard office environment; maintain a stationary position >95% of the time. Operate computer equipment and occasionally move office equipment weighing 10–25 pounds.
