Psychosocial Rehabilitation Services Assistant (PRSA)

Winston Manor – Chicago, IL

Winston Manor is seeking a compassionate, dependable, and organized PRSA to join our team. The PRSA works directly with residents to provide support, guidance, and assistance while promoting independence, personal development, community involvement, and overall quality of life.

This position is ideal for someone who enjoys working directly with residents, building positive relationships, and helping individuals work toward their personal goals in a supportive Intermediate Care Facility environment.

Responsibilities

• Provide direct support and assistance to assigned residents

• Develop positive, professional, and supportive relationships with residents

• Assist residents in working toward individualized goals and greater independence

• Encourage resident participation in programs, activities, and community-based opportunities

• Provide guidance and support with daily routines, social skills, and independent living skills as appropriate

• Monitor resident progress and communicate changes, concerns, or needs to the appropriate team members

• Participate in resident care planning and interdisciplinary meetings as assigned

• Assist with implementing individualized service and treatment plans

• Complete required progress notes, reports, and other resident documentation accurately and timely

• Advocate for resident rights, dignity, choice, and independence

• Assist residents with accessing appropriate facility and community resources

• Communicate effectively with nursing, Social Services, behavioral health, Activities, and other members of the interdisciplinary team

• Support residents during appointments, activities, outings, or other services as assigned

• Maintain resident privacy and confidentiality

• Follow facility policies, procedures, safety practices, and applicable regulatory requirements

• Perform additional resident-support responsibilities as assigned

Qualifications

• High school diploma or equivalent required; additional education in human services, social services, behavioral health, psychology, or a related field preferred

• Previous experience in an ICF, behavioral health, human services, long-term care, or related environment preferred

• Compassionate, patient, dependable, and professional

• Strong communication and interpersonal skills

• Strong organization and documentation skills

• Ability to establish appropriate professional boundaries while building positive relationships with residents

• Ability to work effectively as part of an interdisciplinary team

• Ability to support individuals with varying behavioral, social, and functional needs

• Must meet applicable health and background requirements

Schedule

• Full-time and part-time opportunities may be available

• Schedule may include weekdays, evenings, weekends, and holidays based on resident and facility needs
