Birch Creek Post Acute & Rehabilitation offers around-the-clock skilled nursing in a residential setting. Our community features an outdoor recreation area, a patio garden, a rehab gym, a beauty/barber shop, a library, pastoral support, and transportation services.Large 1000 sqft gym space at south end of building with lots of windows/natural light, ADL Bathroom and Kitchen/Laundry training spaces, OmniVR: 3-D virtual reality system used for strengthening, balance, endurance, coordination, and cognitive skills development.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.

Salary/ Wage Range

Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
