Hybrid Transition of Care Navigator (Behavioral Health)

Molina Healthcare seeks a licensed Behavioral Health Transition of Care Coordinator to support Medicaid members discharged from psychiatric hospitals. This hybrid role combines remote work with local hospital travel to ensure seamless post-discharge care and reduce readmissions.
