Oncall LVN Runner Per Diem

About the Role:

Newport Hospice is looking for a compassionate and dedicated On-Call LVN Runner to join our team in San Bernardino, CA. This is a meaningful opportunity to make a real difference in the lives of patients and families during their most critical moments, providing essential clinical support in a fast-paced, mission-driven environment.

Responsibilities:

Respond to on-call patient needs, providing timely bedside nursing support in home and facility settings
Perform skilled nursing assessments, symptom management, and comfort-focused interventions
Administer medications and treatments as directed by the care plan and supervising RN
Communicate patient status updates clearly to the on-call RN and interdisciplinary hospice team
Provide emotional support and education to patients and families during end-of-life care
Accurately document clinical observations and interventions in the patient medical record
Coordinate with hospice staff to ensure seamless continuity of care during after-hours visits

Requirements:

Active Licensed Vocational Nurse (LVN) license in the state of California
Previous hospice, palliative care, or home health nursing experience preferred
Ability to work flexible on-call shifts, including evenings, weekends, and holidays
Strong clinical assessment and critical thinking skills in high-acuity situations
Reliable transportation and valid California driver's license required
Compassionate, patient-centered approach with excellent communication skills
Proficiency in electronic medical records (EMR) documentation
