Growing together.As an SNBC Care Coordinator, you will make outbound calls and respond to a telephonic member queue and crisis calls. This position combines working remotely at home (~85%) with in-person meetings with members (~15%) and occasional on-site meetings at the corporate headquarters. Residency in or near the Twin Cities, MN metro is required. We offer a comprehensive benefits and rewards package that may include:Competitive salaryGenerous paid time offMedical, dental, vision, and behavioral health benefits401(k) with company matchFamily support and wellness resourcesTuition reimbursement and dedicated learning timeCareer development opportunities across a fast‑growing organizationPrimary Responsibilities:Provide telephonic care coordination to MN Medicaid members with a mental health disabilityComplete annual face-to-face meeting for qualified membersManage gaps in care for members in both their mental health and medical needs (i.e., readmissions to higher levels of care, annual exams/services, etc.)Manage member transitions to and from acute care settings (admissions/discharges)Complete annual Health Risk Assessments and Care Plans for membersCoordinate and collaborate care with providers, facilities, and supportsMake clinically appropriate referrals for membersCoordinate and authorize medically appropriate services for membersAnswer calls on the Special Needs Basic Care (SNBC) member queue and on the crisis lineSchedule and facilitate care conferences with members' providersWork with internal clinical teams to manage members' needs You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Master's or Doctoral Degree in Psychology, Social Work, Counseling, or related fieldActive, unrestricted, independent license in Minnesota: LP, LPCC, LICSW, LMFT2+ years of clinical experience in a behavioral health settingProven ability to work in multiple computer and web-based applications simultaneouslyValid driver's license and access to reliable transportation that will allow you to travel within the Metro for annual member visits (up to 15%) and occasional in-office meetingsLive in or near the Twin Cities, MNDedicated, distraction-free workspace and high-speed internet at homePreferred Qualifications:Case Management experienceExperience managing a caseloadExperience working with Adults with Severe and Persistent Mental Illness (SPMI)Experience with Substance Use Disorders (SUD)Experience working on a phone queue or crisis lineExperience working in a strength-based recovery modelExperience using motivational interviewingExperience making home visitsProven organizational skillsProven ability to work independentlyPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 to $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
