Integrated Services Case Manager - Mental Health & Housing

Sulzbacher Center

A social services organization in Jacksonville is looking for an Integrated Services Case Manager. This role involves providing intensive case management for individuals dealing with mental health and substance abuse issues. Responsibilities include conducting home visits, interacting with clients to facilitate service coordination, and collaborating with community resources.
