Your Role:

As a Home Health PT, you’ll provide in-home physical therapy to patients recovering from injuries, surgeries, or medical conditions, helping them achieve their mobility and wellness goals.

Key Responsibilities:

• Evaluate patient needs and develop custom treatment plans to improve mobility, relieve pain, and enhance function
• Provide therapeutic exercises, manual therapy, and other interventions for recovery
• Educate patients and families on the importance of therapy and support them throughout the recovery journey
• Collaborate with healthcare teams including physicians, nurses, and OTs to ensure seamless care
• Document patient progress accurately to support coordinated care and positive outcomes
• Stay current on professional development and industry best practices

What We’re Looking For:

• Licensed Physical Therapist (PT) in Arizona
• Strong communication skills and the ability to build trust and rapport with patients and colleagues
• Ability to work independently and as part of a team in a dynamic setting
• Analytical and problem-solving skills to adjust treatments for optimal outcomes
