Registered Nurse (RN) - WOCN

Overview The WOCN provides expert care and coordination for patients requiring wound, ostomy, and continence management in the home setting. This role supports high-quality patient outcomes through advanced clinical practice, staff education, and collaboration with the interdisciplinary care team. This position will cover Western Nassau and Queens. 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload Job Details Key Responsibilities Assess, plan, implement, and evaluate patient care in the home setting Provide specialized wound, ostomy, and continence treatments Monitor patient progress and adjust care plans as needed Serve as a clinical resource and mentor to nursing staff Supervise Home Health Aides and support care team performance Ensure accurate documentation and compliance with regulatory standards Collaborate with physicians and healthcare providers to ensure continuity of care Participate in quality improvement and patient outcome initiatives Qualifications Bachelor’s Degree in Nursing (BSN) required Current New York State Registered Nurse (RN) license Minimum 5 years of community health nursing experience Strong clinical expertise in wound, ostomy, and continence care CWOCN (Certified Wound, Ostomy, and Continence Nurse) required Excellent communication, organizational, and problem‑solving skills Salary Range USD $105,000.00 - USD $136,500.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
