As a Home Health Physical Therapist Assistant, you'll enjoy the autonomy of managing your schedule, providing one-on-one care in patients' homes, and helping individuals regain strength, mobility, and independence where they are most comfortable.

Responsibilities

What You'll Do

• Provide skilled physical therapy treatments under the direction and supervision of a Physical Therapist in accordance with physician orders and individualized plans of care.
• Assist patients in improving mobility, strength, balance, endurance, and overall functional abilities.
• Implement treatment plans designed to help patients achieve their rehabilitation goals.
• Document patient treatments, responses, and progress in a timely and accurate manner.
• Collaborate with Physical Therapists, physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
• Educate patients and caregivers on exercises, safety techniques, and home programs.
• Communicate changes in patient condition and progress to the supervising Physical Therapist.

Qualifications

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in Pennsylvania.
• Current CPR certification (or ability to obtain upon hire).
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong communication, organizational, and documentation skills.
• Home health experience preferred, but not required.
