Job Summary:

A licensed Physical Therapist (PT) is responsible for evaluating and implementing treatment plans to improve residents' mobility, strength, and overall physical function. They collaborate with a healthcare team to deliver high-quality, personalized care that promotes recovery and independence.

Responsibilities:

• 
  Conduct comprehensive assessments of patients physical capabilities and develop tailored treatment plans.
  
  
• 
  Deliver evidence-based physical therapy interventions to enhance mobility and functional abilities.
  
  
• 
  Educate patients and families on exercises, movement safety, and home-care modifications.
  
  
• 
  Coordinate with interdisciplinary team members to ensure seamless patient care and optimal outcomes.
  
  
• 
  Monitor patient progress, adjusting treatment strategies as needed to meet recovery goals.
  
  
• 
  Document assessments, treatment sessions, and progress notes accurately in compliance with facility policies.
  
  

Qualifications:

• 
  Hold a valid Physical Therapy license in the state.
  
  
• 
  Possess a Bachelors, Masters, or Doctoral degree in Physical Therapy from an accredited program.
  
  
• 
  Preferably have experience working in a skilled nursing facility or similar healthcare environment.
  
  
• 
  Demonstrate knowledge of therapeutic modalities, adaptive equipment, and exercise techniques.
  
  
• 
  Exhibit strong communication, interpersonal, and problem-solving skills.
  
  
• 
  Show a compassionate, patient-centered approach with the ability to work independently and as part of a team.
