RN Case Manager — Discharge Planning & Care Coordination

Inova Fair Oaks Hospital is seeking a dedicated Registered Nurse Case Manager to join the Case Management Team. This Part-Time Day shift (20 hours per week) runs 8:00 AM to 4:30 PM with rotating weekends and one major and one minor holiday. The RN Case Manager I oversees discharge planning and coordination of care in acute and post-acute settings.
