Aging Life Care Specialist

Aging Life Care Specialist (Geriatric Care Manager) We are hiring an Aging Life Care Specialist at Aging Life Care Association®. The position is full-time, 40 hours a week with on‑call responsibilities one week per month or part‑time hours. The role offers the opportunity to advance to a supervisory position for the right individual. The compensation is hourly plus mileage, and health insurance is available after the probationary period with the employer compensating part of the premium. Responsibilities: Client visits and assessments regarding resources and levels of care. Developing and monitoring care plans. Coordinating physician appointments. Advocating for clients and staying familiar with local senior resources. Managing own caseload as a self‑starter who enjoys advocacy. Qualifications: MSW, BSW, CMC, RN, LPN, or LF degree or certification. Clinical experience with elderly population and mental health preferred.
