Outpatient Physical Therapist

We are looking for an Outpatient Physical Therapist to join our rehabilitation team at the Joliet, IL clinic. Your role is to plan, implement and evaluate patient care programs designed to increase, maintain or restore function, minimize or prevent disability and assist patients in reaching maximum performance levels. Essential Duties and Responsibilities Evaluates patients and gathers appropriate information to determine accurate clinical diagnoses and treatment plans. Establishes appropriate, functional and objective goals based upon clinical examinations. Devises detailed treatment and care plans for patients. Provides exceptional patient care and service. Maintains friendly, cordial relations with clients and employees; fosters a positive work atmosphere. Cultivates relationships with patients and the community. Provides continuous communication with referring physicians and other referral sources to ensure consistent care. Maintains patient charts, clinical logs and checklists necessary for compliance. Maintains patient confidentiality regarding medical records per company policies and procedures and assures compliance with all HIPAA guidelines. Communicates well in a professional setting. Performs additional duties as required. Education and Experience Bachelor’s, Master’s or Doctorate Degree from an accredited Physical Therapy Program. Current State of Illinois Physical Therapy license (or willingness to apply). CPR Certified. Compensation and Benefits Pay Range: $80,000 - $100,000 annualized.
