Crisis Response Case Manager (Bilingual)

Pillars Community Health, LLC

Pillars Community Health, LLC is seeking a Case Manager to support the Mobile Crisis Response team. You will assist clients with hospital engagement, discharge planning, crisis stabilization, and linkage to post-hospital and outpatient services.
