Location: Clemmons, North Carolina

We are seeking a dedicated and compassionate Physical Therapist (PT) to join the team at Cedar Hills Center for Nursing & Rehabilitation! If you’re passionate about helping residents improve mobility, regain independence, and enhance their quality of life, we’d love to hear from you.

The licensed Physical Therapist evaluates and treats patients and communicates with patients, clients, and team members. The therapist maintains documentation of services in the medical record and conducts ongoing in-services and training for patients, clients, and team members on restorative techniques.

• Supervise Therapist Assistants and therapy students in direct patient care, ensuring treatment plans and patient-related activities comply with state practice acts and reimbursement regulations.

• Maintain a positive attitude and promote a collaborative, team-oriented environment.

• Maintain consistent attendance and provide appropriate notice for scheduled paid time off.

• Meet productivity standards established by the supervisor and clinical needs.

• Demonstrate knowledge and proper application of Medicare and other reimbursement regulations.

• Evaluate patients based on physician referrals and orders.

• Develop effective, goal-oriented treatment plans and obtain required physician approvals and orders.

• Provide treatment according to established care plans.

• Maintain accurate clinical documentation in accordance with regulatory and professional guidelines.

• Record daily treatment charges according to approved billing guidelines.

• Instruct families and nursing staff on restorative and home exercise programs.

• Participate in discharge planning and interdisciplinary team meetings.

• Communicate with supervisors and healthcare team members regarding patient progress, challenges, and treatment plans.

• Participate in patient care conferences, utilization review meetings, and other required meetings.

• Provide in-service training for facility staff and participate in performance improvement programs.

• Coordinate care with other departments, attending physicians, and rehabilitation team members.

• Screen patients based on identified needs.

• Maintain a clean and safe treatment area and report equipment issues.

• Obtain necessary durable medical and adaptive equipment for patients.

• Travel or float between facilities as directed by the supervisor and patient care needs.

• Perform other duties as assigned.

• Graduate of an accredited college or university.

• Valid state Physical Therapy license or eligibility to obtain a license in the state where services are provided.

• Ability to walk, stoop, bend, stand, sit, push, pull, and lift throughout the workday.

• Ability to communicate effectively in English.

• Ability to manage the mental and emotional demands of the position.

• Adequate vision and hearing, with or without assistive devices.

• Ability to work independently and effectively with patients, families, and team members.

• Good general health, emotional stability, and personal integrity.

• Ability to work with elderly, ill, disabled, and emotionally distressed individuals.

• Ability to lift patients, medical equipment, and supplies weighing up to 50 lbs.

• Work in well-lit and ventilated office and therapy areas.

• Frequent sitting, standing, bending, lifting, and moving throughout the workday.

• Frequent interruptions and interaction with patients, families, and staff in a variety of circumstances.

• Willingness to work beyond normal hours or temporarily assist in other positions when necessary.

• Maintain constant awareness of patient safety.

• Attend continuing education programs, which may involve overnight travel.

• Exposure to potential workplace hazards, including infectious diseases, falls, and other environmental risks.

• Ability and willingness to travel between facilities as directed.

• Personal phone use is permitted only during breaks and unpaid lunch periods; phones must remain on vibrate during working hours.

Supervision Received: Reports to the Director of Rehabilitation or designee for administrative and patient care responsibilities.

Supervision Exercised: Supervises Therapist Assistants, rehabilitation technicians, temporary or provisional licensees, and therapy students.

Exposure Risk: The position carries a high risk of exposure to blood and bodily fluids.
