Home Health RN - Registered Nurse Multiple Shifts

Job Title: Registered Nurse – RN Location: Knoxville, TN Job Type: Full-Time, 7 on / 7 off POSITION OVERVIEW The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. RESPONSIBILITIES Coordinate the total plan of care with other health care professionals, maintain continuity by planning and exchanging information with provider, agency personnel, patient, family, and community resources. Make initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop plan of care within state guidelines, and ensure patients meet home health eligibility and medical necessity criteria. Maintain and submit accurate documentation and clinical notes, including patient’s condition, goals, interventions, and response to care. Provide high-quality clinical services within scope of practice as defined by state laws, from admission through discharge. QUALIFICATIONS One (1) year of clinical RN experience. Current RN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and dependable vehicle or public transportation to commute to multiple patient residences daily within the geographic area assigned.
