Role Overview As a Case Manager, you play a vital role in our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. You'll assess, plan, and facilitate care transitions, focusing on safety, quality, and cost-effective outcomes. Your work directly impacts patient experiences, helping them navigate critical health events and access necessary resources along the care continuum. What You Will Do Identify and assess patient needs, developing care plans that promote safety and quality. Coordinate care transitions to ensure a safe and effective move to the next level of care. Advocate for patients and families, connecting them with community resources and support services. Collaborate with interdisciplinary teams to optimize health outcomes and manage complex patient needs. Evaluate care plans and outcomes, making adjustments to meet evolving patient requirements. Qualifications What You Will Need Required Qualifications Bachelor's degree in nursing or a Master's degree in Social Work. Minimum 3 years of clinical experience. Excellent verbal communication, written documentation, and interpersonal skills. Strong problem-solving, analytical, and decision-making abilities. Proficiency in computer skills and knowledge. Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Preferred Qualifications Experience in discharge planning, home health care, rehabilitative medicine, or managed care. Certification in Case Management (CCM) or Accredited Case Manager (ACM).
