Responsibilities

• Conduct comprehensive physical therapy evaluations for residents in a skilled nursing facility.
• Develop and implement individualized plans of care to restore mobility, strength, balance, and function.
• Provide therapeutic interventions, including gait training, therapeutic exercise, ROM, strengthening, and balance training; select appropriate modalities as indicated.
• Monitor patient progress, document outcomes, and adjust treatment plans in collaboration with the interdisciplinary care team.
• Educate residents and caregivers on home exercise programs, safety, and strategies to maintain function after discharge or transfer.

Requirements

• Prior experience working in a skilled nursing facility is required.
• Ability to work a daytime, Monday–Friday schedule (8:00 AM–4:30 PM).
• Willingness to participate in a 12-week travel assignment.
• Strong documentation and communication skills for care coordination.
